As a leading local resource for dementia support and education, they offer free 24/7 helpline access (1-800-272-3900), support groups (in person and virtual), caregiver education, and referral to programs such as the state’s caregiver support initiatives.

Serving the Alzheimer’s Association mission to help those in need ensures we can connect them (and their caregivers) to trusted resources for education, emotional support, care planning, and community services — going beyond what a single primary-care clinic can offer.
Memory Care Home Solutions (MCHS) delivers completely free dementia-care support directly to patients and families in the comfort of their own home. Their dementia specialists spend the time needed to understand each patient’s daily challenges and routines, offering personalized guidance on safety, behavior changes, communication strategies, and long-term planning.
Patients benefit from more consistent attention, more hands-on support, and care that adapts to their home environment, not the other way around. Each family is paired with a dedicated Care Navigator — a dementia-trained professional who follows them throughout the entire journey.
