Please note our hours of operation are Monday-Friday 8:00am-5:00pm CST. We are closed on weekends & all major US holidays. All appointments must reviewed & approved by our team prior to scheduling. We do not accept walk-ins or solicitations. Once you have selected a time & date for which we are available per the calendar below, please submit the scheduling request form located beneath the calendar. We will respond to all scheduling requests in the order they were received. Please respect that patient scheduling needs will take precedence over all business discussions.
