Mental Health & Emotional Wellness Care

Be Your Best Self

Our Approach

At Guardian Primary Care, we provide comprehensive mental health support as part of primary care. This allows patients to address emotional health concerns in a familiar, accessible setting without unnecessary fragmentation of care.

We evaluate, diagnose, and manage a wide range of mental health conditions, including but not limited to:

  • Depression and mood disorders
  • Anxiety disorders
  • ADHD (when clinically appropriate)
  • Sleep-related concerns
  • Stress-related and adjustment disorders

Care is individualized based on symptoms, clinical history, and evidence-based guidelines. When appropriate, we may also utilize validated screening tools and laboratory testing to rule out or address contributing medical conditions.

Treatment decisions are made collaboratively between patient and provider, with a focus on safety, function, and long-term well-being.

Integrated Care & Referrals

Mental health care is not one-size-fits-all. When clinically appropriate, we may:

  • Initiate and manage mental health medications within primary care
  • Adjust or stabilize existing psychiatric medications
  • Coordinate care with licensed counselors and therapists
  • Refer to psychiatry for higher-acuity, complex, or treatment-resistant conditions
  • Connect patients with community mental health resources and support services

Our goal is to ensure patients receive the right level of care at the right time, whether within our clinic or through trusted external specialists.

Crisis Support & Safety

If you are experiencing a mental health crisis, or if you or someone you know may be at risk of harming themselves or others:

  • Call or text 988 to reach the Suicide & Crisis Lifeline (available 24/7)
  • Or go to the nearest emergency department immediately
  • If there is immediate danger, call 911

You are not alone, and help is available at all times.

Crisis Support & Safety

What to Expect

Step 1 — Evaluation

We begin with a comprehensive assessment focused on understanding your symptoms, history, and contributing factors.

This may include:

  • Mood, anxiety, and attention symptoms
  • Sleep patterns and stress load
  • Medical history and current medications
  • Substance use screening when appropriate
  • Targeted laboratory testing if medically indicated

Step 2 — Personalized Treatment Plan

If treatment is appropriate, we develop a plan tailored to your needs. This may include:

Medication Management (when indicated)

We may prescribe and manage:

  • Antidepressants
  • Anti-anxiety medications
  • Sleep aids (short- or long-term when appropriate)
  • ADHD medications (when clinically indicated)

All medications are prescribed based on clinical evaluation, current guidelines, and safety considerations.

Counseling & Therapy Coordination

We strongly support therapy as a core component of mental health care and may:

  • Refer patients to licensed therapists or counselors
  • Coordinate care with behavioral health providers
  • Provide local and regional counseling resources
  • Assist patients in finding appropriate long-term therapy support
Psychiatric Referral

When higher-level care is needed, we coordinate referrals to psychiatry for:

  • Complex medication management
  • Severe or treatment-resistant conditions
  • Diagnostic clarification
  • Specialized psychiatric evaluation

Step 3 — Follow-Up & Ongoing Support

Mental health care requires ongoing monitoring and adjustment.

Typical follow-up includes:

  • Early follow-up within 2–6 weeks after starting or adjusting medication
  • Routine visits every 1–3 months during active management
  • Stabilized patients seen at longer intervals as appropriate

We monitor:

  • Symptom improvement and functional outcomes
  • Medication effectiveness and side effects
  • Sleep, mood, and daily functioning
  • Safety concerns or emerging risks

Our Approach to Care

We believe in:

  • Treating the whole person, not just symptoms
  • Starting with the lowest effective intervention
  • Using evidence-based guidelines
  • Prioritizing safety and long-term stability
  • Coordinating care across providers when needed
  • Ensuring patients feel heard, supported, and respected

What You Can Expect

You can expect clear communication, structured care, and a supportive clinical environment. Our goal is to help you stabilize symptoms, improve daily functioning, and build long-term emotional resilience.

If mental health treatment is appropriate, we will guide you through each step and ensure you are connected to the right level of care.
